Tracing

Health issues are a private matter, so reporting to the club is not mandatory.  However, to help manage the safety of the club, we ask WPRC participants to notify the COVID-19 Coordinator if any of the following occur:

Event

Information Requested

  • Receive a positive COVID-19 test
  • Name of patient
  • Test date
  • Date of first symptoms
  • Required to quarantine due to:
    • COVID-19 symptoms
    • Close contact
    • Travel requirement
  • Name of patient
  • Date of last contact or return from travel
  • Date of first symptoms

All information will be kept private.  Names will not be revealed to those with whom you came in contact.  All internal reporting will be done at an aggregate level.

By providing this information in a timely manner, the tracking and tracing of club activities will be facilitated and expedited.

In order to understand how the information will be used, the following are example communications that we intend to use in the case of a reported positive COVID-19 test.

Example Communications for Reported Positive COVID-19 Test

To All of WPRC

WPRC,

An individual in the [age group]'s tested positive for COVID-19 on [date of test].  Symptoms for the individual started on [first date of symptoms].  The individual attended the following WPRC events:

  • [date of training] training sessions - AM and PM

We will send separate communications directly to individuals who were in attendance with the individual.  If you have any questions or concerns, please reach out to the me.

To Close Contacts

[Parents of athlete],

An individual in the [age group]'s tested positive for COVID-19 on [date of test].  Symptoms for the individual started on [first date of symptoms].  The individual attended the following WPRC events:

  • [date of training] training sessions - AM and PM

According to WPRC records, [name of athlete] was in attendance with the individual.  Your athlete has been identified as a COVID-19 “close contact” and should follow the “close contact” guidelines for quarantining.

If your child was not in attendance, please notify me.  If you have any other questions or concerns, please reach out to the me.
